Description & Eligibility: The Chase Paperless Statements Galapagos Sweepstakes (the "Sweepstakes") begins on March 1, 2008 12:00:01 AM
EST and ends on April 30, 2008 11:59:59 PM EST ("Promotion Period"). This Sweepstakes is offered only to legal residents of the 50 United States and the
District of Columbia, 18 years of age and older during the Promotion Period. Employees of Chase Bank USA, N.A. (the "Sponsor") and their immediate families
(i.e. spouse, parents, children, siblings, and their respective spouses), participating prize partners, their respective parent companies, affiliates,
subsidiaries, divisions, advertising and promotion agencies, and employees of World Wildlife Fund, Inc. are not eligible to enter. Sponsor reserves the
right to disqualify participants who violate these official rules ("Official Rules") or interfere with this Sweepstakes in any manner. If a participant is
disqualified, Sponsor reserves the right to terminate that participant's eligibility to participate in this Sweepstakes. This Sweepstakes is subject to all
federal, state, and local laws. Void in Puerto Rico and where restricted or prohibited by law.
HOW TO ENTER: Current Chase OnlineSM customers who are enrolled in Paperless Statements with a credit card account from Chase Bank USA, N.A. at the start of
the Promotion Period will automatically earn an entry into the Sweepstakes if they are still enrolled in Paperless Statements at the end of the Promotion
Period.
For customers who are not currently enrolled in Paperless Statements from Chase, there are two ways to enter. To enter online: during the Promotion Period log onto http://galapagos.chasesweepstakes.com. New and current eligible Chase Credit Card Account customers who enroll or are enrolled in the Chase OnlineSM service and who enroll in Paperless Statements by the end of the Promotion Period will receive one automatic entry into the Sweepstakes. Enrollments made under multiple identities or through the use of any agent or other device or artifice to enter multiple times will be deemed invalid. To enter by mail without enrolling or without having a credit card account: print your name, street address, city, state, zip code, telephone number, date of birth and email address on a 3" x 5" piece of paper, place in envelope and mail to: Chase Paperless Statements Galapagos Sweepstakes, P.O. Box 6000, Manhasset, NY 11030-6000. Limit of one (1) entry per person regardless of method of entry. Mail-in entries must be postmarked by April 30, 2008 and received by May 7, 2008 to be eligible.
PRIZE: One (1) Grand Prize: An eleven (11) day vacation for two (2) on one of Natural Habitat Adventures' ("NHA") scheduled Classic Galapagos Islands Cruises on board the M/Y Letty. Prize includes round trip coach air transportation from the major international airport nearest the Winner's home, accommodations, meals from breakfast on Day 2 to breakfast on final day, except for 1 lunch and 1 dinner in Quito, services of NHA's professional Expedition Leaders, airport and hotel transfers, use of snorkel gear and wetsuits, Otavalo Market excursion (except on family departures). Prize does not include any additional expenses, such as, but not limited to: gratuities, beer and liquor at hotels and on board M/Y Letty, Galapagos tax and transit card (approx. $110 for adults, $60 for children under 12, collected in advance), airport departure tax (approx. $40), items of a personal nature (phone calls, laundry, etc.), travel insurance, optional activities. Travel arrangements must be made through Sponsor's travel agent, Promotion Administrator, and/or NHA and on a flight and carrier and hotel of Sponsor's choice. Travel must be completed by December 31, 2009 and is based on availability at the time of booking. Winner and guest agree to abide by all of Natural Habitat Adventures' Terms and Conditions as a condition of travel. Certain travel restrictions may apply. Travel companion must be 18 years of age or older or a member of Winner's immediate family. Winner and guest must travel together on the same itinerary, or prize will be forfeited. In addition, the Winner and his/her travel companion are responsible for obtaining all required travel documents, as well as travel insurance (and all other forms of insurance) that they may wish to obtain (at their own expense) and hereby acknowledge that the Sponsor has not and will not obtain or provide travel insurance or any other form of insurance. It is the responsibility of the Winner and his or her travel companion to arrive at the designated U.S. gateway city in time for their flights, with proper travel documentation, including passports or visa, if necessary. Certain travel and security restrictions may apply to travel generally and to the Winner and his or her companion specifically. Sponsor is not responsible for any acts of nature that might result in travel being prohibited. Prize is not transferable and cannot be assigned, redeemed for cash, substituted (except by Sponsor, in its sole discretion, to substitute a prize of equal or greater value if advertised prize is unavailable) and will not be replaced if tickets, etc. are lost, stolen, or misplaced by Winner or if booked transportations are missed for any reason. RANDOM DRAWING/NOTIFICATION: Winner will be selected in a random drawing on or about May 11, 2008 by Marden-Kane, Inc., an independent judging organization from among all eligible entries received. Winner will be notified by phone, email, US mail, and/or overnight courier. Winner may be required to execute and return an Affidavit of Eligibility and Liability Release and where legal, a Publicity Release within 14 days following the date of issuance of notification. Winner will also be required to furnish his/her social security number that will be used only for the purpose of reporting the Winner's prize earnings to the Internal Revenue Service, as required by law. Winner and the guest of Winner must complete a Liability Release before any travel documents are prepared or issued. Failure to timely return any documents requested, return of prize notification as undeliverable, or noncompliance with these Official Rules will result in such potential Winner being disqualified. Sponsor shall have no further obligation to that potential Winner, and an alternate Winner will be selected. The odds of winning a Prize depend on the total number of eligible entries received via all entry methods. In no event will more prizes be awarded than those listed in these Official Rules.
